DISTRICT ATHLETIC PHYSICAL
EXAMS
District Athletic Physicals for 2008-2009:
Tues., June 3, 2008
6- 8pm Kamiak Gyms
$25.00 Cash or checks accepted.
Checks to be made out to Kamiak High School.
Incoming ninth graders must have a new physical and another one
in their junior year. All physicals are good for two years.
Any questions about an athlete's physical may be directed to
Mrs. Billerbeck in the athletic office at 425-356-6625.
Important information
regarding sports physicals
It
has been brought to our attention that sports physicals obtained
through some clinics will only be good for one year. WIAA honors
physicals for two years. Please check with the athletic office if
you are unsure of your physical expiration date and check with your
physician before making an appointment that the physical will be
good for two years. The physicals taken through the high schools in
the spring are good for two years.
College-bound athletes planning on playing
at
Division 1
and Division 2 schools
need to complete the
NCAA
Clearinghouse
application online at www.ncaaclearinghouse.net

SPORTS CLEARANCE:
Athletic
registration forms
and
physical exam forms may be dropped off in the
athletic or main office. All
athletes need clearance for a sport to turn out.
Students must have a 2.0 GPA, no F’s, current
physical, parent permission, and insurance to be cleared. An ASB
card is required by the first contest.
