Kindergarten Registration

Odyssey offers a half day kindergarten program.  Parents can choose a morning or afternoon session.  Hours of morning kindergarten are 8:55 – 11:32.  Hours for the afternoon session are 12:53 – 3:30.  Registration for the following school year begins in March.  Registration packets can be picked up at the school office.  In order to register for kindergarten, the following documents are required:

• Proof of birth (birth or hospital certificate)
• Proof of residence (lease agreement or utility bill with your name and address)
• Up to date immunization records

Note to Kindergarten Parents

All immunizations must be up to date and a copy of the proof of birth must be submitted with the registration materials.  Students will not be placed on a class list until these documents are received and approved by the office. 

Be sure to list parents as emergency contacts in section two of the white enrollment form.  Please provide contact information for at least two other adults (relatives, friends, neighbors) we can call in an emergency when we cannot reach you.  Your child can be released only to the adults you list in the contact section.

Todos registros de inmunizacion de de deben depender de fechar y una copia de la prueba de nacimiento debe estra presentado con los materiales de registro.  Los estudiantes no sera situado en una lista de clase hasta estos documentos estan recibido y estan aprobado por la oficina.

Asegurense de listar padres como contactos de emergencia en seccion dos de la forma de alistamiento blanca.  Por favor provee informacion de contacto para al menos dos otros adultos (familiares, amigos, vecinos) podemos hacer entrart una emergencia cuando no podemos alcanzarle. Nuestro nino puede estar liberado solamente para los adultos que usted lista en la seccion de contacto.

Paperwork Instructions:

Proof of Birth - Required
Please bring proof of your child's birth, so our office can take a copy.  This is required for Kindergarten.

Proof of Residency
You can use a lease agreement or a bill from your phone or electric company.  Your documentation must show your current address is within our school's service area.

White Folder
This is included in the packet you will get at our school's office.  The top three boxes will need to be filled out.  Please make sure you have your current address, phone number, and contact information all parents and guardians.

If your child has health concerns for us to be aware of, please bring this information with you as well.

Registration Form
When you fill out the registration form, you will need the following information:

  
 Immunization
The State of Washington requires students to have all shots before they are allowed in school.  You will need to provide dates for the following immunizations:

A Pupil Health Card, Kindergarten Questionnaire, and Kindergarten Identification Name Tag may also be required at registration.
